A child with a disability must be provided substitutions in foods when that need is supported by a signed statement from a licensed physician.<\/p><p><a href=\"https:\/\/web.risd.org\/childnutrition\/wp-content\/uploads\/Child-Nutrition-Special-Diet-Form-09212022_.pdf\">Click here for the Child Nutrition Special Diet Form (PDF)<\/a><\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b4010cf elementor-widget elementor-widget-heading\" data-id=\"b4010cf\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">In Cases of Food Allergy<\/h2>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-a77a91e elementor-widget elementor-widget-text-editor\" data-id=\"a77a91e\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p>When a licensed physician\u2019s assessment notes that a food allergy may result in severe, life-threatening (anaphylactic) reaction, the child\u2019s condition would meet the definition of \u201cdisability\u201d. The licensed physician must prescribe the substitutions that need to be made to the meals. Children with food allergies that are non-life threatening may also be considered to have a disability and may also require a meal modification.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-68bff80 elementor-widget elementor-widget-heading\" data-id=\"68bff80\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Medical Statement for Children with Special Dietary Needs<\/h2>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-31e4d4f elementor-widget elementor-widget-text-editor\" data-id=\"31e4d4f\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p>Each special dietary request must be supported by a statement explaining the requested food substitution and must be signed by a recognized medical authority. The Medical Statement must include:<\/p><ul><li>An identification of the medical or other special dietary condition which restricts the child\u2019s diet;<\/li><li>The food or foods to be omitted from the child\u2019s diet; and<\/li><li>The food or choice of foods to be substituted.<\/li><\/ul><p>If we do not receive a medical statement from a recognized medical authority, your child will receive a regular lunch tray.
