data-id=\"0b14630\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\tIn accordance with law, Richardson ISD must provide parents with written notice of each school-based health-related service offered at the campus their child attends. These routine services promote student safety, wellness, and readiness to learn. The services may be provided by qualified school staff, including nurses and athletic trainers. This consent does not take the place of an individualized health plan, 504 plan, or other legally required document.\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-21fe764 elementor-widget elementor-widget-text-editor\" data-id=\"21fe764\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\tA parent has the right to withhold consent for or decline any health-related service. Consent given through this form is effective during the current school year unless revoked earlier.\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-a966058 elementor-widget elementor-widget-heading\" data-id=\"a966058\" data-element_type=\"widget\" id=\"services\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\"><a href=\"#prevent\">Routine Health-Related Services Provided at Your Child\u2019s Campus may include:\n<\/a><\/h2>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c2afb9f elementor-widget elementor-widget-text-editor\" data-id=\"c2afb9f\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<ul><li>First Aid Care (e.g., cleaning and bandage)<\/li><li>Application of topical antiseptics and anti-itch (e.g., S.T.37, calamine lotion, vaseline)<\/li><li>Eye irrigation for foreign particles<\/li><li>Dental first aid (e.g., application of gauze for a lost tooth)<\/li><li>Temperature checks using a non-invasive thermometer<\/li><li>Evaluation or assessments of general illness symptoms (e.g., stomachache, headache)<\/li><li>Evaluation or assessment of student injuries<\/li><li>Use of non-pharmacological interventions (e.g., ice packs, cold compress, heating pad, peppermint)<\/li><li>Monitoring of chronic conditions (e.g., asthma, diabetes)<\/li><li>Promotion of health and wellness (e.g., increasing water intake)<\/li><li>Annual Vision, Hearing, Spinal, and Type 2 Diabetes State Mandated Screenings<\/li><li>Lice Screening<\/li><li>Support during illness or physical symptoms at school<\/li><li>Coordination of health services<\/li><li>Neurological Assessment<\/li><li>Heat illness prevention and injury support for student athletes<\/li><\/ul>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-ac72c5e elementor-widget elementor-widget-text-editor\" data-id=\"ac72c5e\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\tHealth Services may provide the following with additional parental written consent, which requires completing an additional form for each:\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-7b1678e elementor-widget elementor-widget-text-editor\" data-id=\"7b1678e\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<ul><li aria-level=\"1\">Administration of prescribed medication;<\/li><li aria-level=\"1\">Administration of over-the-counter medication;<\/li><li aria-level=\"1\">Providing medically-required procedures.<\/li><\/ul>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-a23506a elementor-widget elementor-widget-heading\" data-id=\"a23506a\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\"><a href=\"#onlineform\">Complete Your Opt In Form Online<\/a><\/h2>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-ded7f8a elementor-widget elementor-widget-text-editor\" data-id=\"ded7f8a\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<a href=\"https:\/\/focus.risd.org\/focus\/index.php\"><strong>Parents should complete their consents by logging on their Focus Parent Portal<\/strong><\/a>.
